Spanish Resources Description
Dyspnea Brochure - English and Spanish This brochure is a guide for people with congestive heart failure or chronic obstructive pulmonary disease.
My Diabetes Diary (Spanish) A tool to assist patients with diabetes to manage their disease. This tool, provided in Spanish, has been translated and made available by TMF Health Quality Institute.
My Emergency Care Plan (Spanish) My Emergency Care Plan in Spanish. This tool, provided in Spanish, has been translated and made available by TMF Health Quality Institute.
Spanish Emergency Care Plan The 2006 VNSR-VNS Emergency Care Plan in Spanish. Revised. Length: one page
Spanish Zones Tools for Asthma, Heart Failure, COPD, and Diabetes These tools are the Spanish translations of the disease management zones which provide patients with easy to follow guidelines for assessing signs and symptoms of their disease process and exacerbations. The tools also advise patients on when to report problems to home health agencies and physicians. The tools include disease management for asthma, diabetes, COPD, and heart failure.
Surgical Wound Care Self-care Book (Spanish) A book translated to Spanish and made available by TMF Health Quality Institute. This book helps the patient understand self-care of surgical wounds.
